About the service
Norfolk and Waveney Community Support Service provides short-term, practical support for people.
We support:
Patients being discharged on Pathway Zero to help them return to the community safely
People in the community to help with temporary issues and avoid escalation and potential hospital admittance.
Support is short-term (typically 2 – 4 weeks but we can extend this according to need).
After referral, if someone is assessed by the service as needing longer-term but temporary support this can be provided for up to 12 weeks.
Support is provided by a team of volunteers and staff across Norfolk and Waveney. When we recruit our volunteers, we undertake safer recruitment checks. The volunteers are DBS checked and trained in data protection, safeguarding, health and safety, and lone working.
The providers
The service is provided by Voluntary Norfolk, British Red Cross and Age UK Norwich working in partnership.
Together we are established voluntary organisations with strong local links bringing experience across Norfolk and Waveney in delivering community support and hospital discharge services. We use staff and volunteer models, linking people with local networks and groups to support them to be confident, resilient and engaged with their community.
The Norfolk and Waveney Community Support Service is commissioned by the Norfolk and Waveney ICB, Norfolk County Council and Suffolk County Council.
